Who Should Consider the HealthShare + DPC Membership?

How It Works

The Primary Care + HealthShare solution has two main components:

1. A direct primary care membership with a leading DPC practice in your area

2. An affordable medical cost sharing membership from Zion HealthShare, one of the most reputable health sharing organizations in the country

Here’s a closer look at each of these components, and how they work together to provide affordable healthcare access across the full spectrum of medical care. 

Direct Primary Care

Direct primary care doctors don’t take insurance – and patients don’t need it. 

Instead, patients pay a flat monthly membership fee to their doctor for a range of primary care services –– similar to a gym membership.

As a DPC patient, you’ll receive an unlimited number of primary care and wellness appointments with your DPC doctor. The doctor will provide whatever primary care is necessary to address your medical needs––with no additional out-of-pocket costs*.

DPC services include basic family medical care normally provided by a generalist in a doctor’s office setting. 

You see your direct primary care doctor for small, routine, and preventive care visits that are normally handled in a doctor’s office. Your monthly subscription includes as many visits as necessary to manage your care.

There are no additional deductibles, copays, or coinsurance. As long as you pay your predictable and affordable monthly subscription, there are no other cost barriers to seeing your primary care provider. 

Note: DPC memberships focus on routine and preventive care. They don’t include hospital or ER visits, surgeries, or specialist care. That’s what the health sharing component of the solution is for. The DPC component strictly focuses on the primary and preventive care pieces.

*If your doctor needs outside services such as lab work or imaging done, these outside organizations may charge a fee. Your Initial Unshareable Amount (IUA)

For larger unexpected medical bills for services not included in your DPC membership, you’ll pay an initial unshareable amount (IUA). The IUA applies per medical need, not per year.

The IUA is the amount you pay on your own before associated eligible medical expenses may be shared through your health sharing membership.

You can choose an IUA of $1,250, $2,500, or $5,000.

The higher your IUA, the lower you can expect your monthly costs to be, all other things being equal. 

Health Sharing

The health sharing component supports you with large eligible medical needs. 

Once you have paid your IUA for a medical need, eligible medical expenses are shared. The IUA applies per medical need, and sharing is based on the date of service.

So your DPC doctor handles routine care, family doctor matters, basic pediatric matters (with some DPCs), checkups, wellness visits, medication updates, minor colds, flus, and scrapes and sprains, and other minor urgent care issues that don’t require special equipment or additional staff.

Your health sharing membership is there for ER visits, hospitalizations, surgeries, advanced imaging (MRIs), and other larger eligible medical expenses.

How to Enroll in Primary Care + HealthShare

Enrolling in the Primary Care + HealthShare membership is simple and straightforward:


  1. Complete the online enrollment form. 
  2. Choose your membership. Select the Zion HealthShare membership option with an Initial Unshareable Amount (IUA) that best matches your healthcare needs and budget.
  3. Select a DPC provider. Select a Direct Primary Care (DPC) provider in your area that partners with Zion HealthShare. Our Personal Benefits Manager can assist you in identifying a participating practice for you.
  4. Enter your payment information. Complete your membership applications for both the DPC practice and the Zion HealthShare membership.
  5. Complete the signup process. Once enrolled, you’ll have access to the DPC services and health sharing support that work together to provide coordinated, cost-effective care.

Employee Group Plans

Employer Considerations

There are a few major considerations employers should be aware of, however:

Taxable to Employees

Unlike traditional group health insurance, the value of the health sharing benefit is taxable to employees.

It’s still fully deductible as an employee compensation expense to the business, though. 

No Prescription Drug Insurance Programs

The Zion HealthShare memberships include access to Zion RxShare, a prescription discount program. However, it does not have a prescription drug insurance component.

This may be a factor if a plan beneficiary needs an expensive brand name drug that’s not available as a generic.

Pre-Membership Medical Condition Waiting Periods

Zion HealthShare has a waiting period before it will share expenses related to pre-membership medical conditions.

So this membership may not be suitable for some employees or family members who have significant pre-membership medical conditions or ongoing medical needs. 

Not HSA-Eligible for DPC Members

Members who are also members of a DPC may not contribute new money to health savings accounts.

However, if they already have balances in their HSA accounts, they can still use their HSA balances to pay for qualified medical expenses tax-free.

Frequently Asked Questions 

Primary Care + HealthShare FAQ IconQ:  Is this a health insurance plan?

A: No. Health sharing programs are not insurance products.

They are not structured or regulated the same way as health insurance plans.

Health sharing programs are non-profit, voluntary mutual aid associations in which like-minded members agree to help share the expenses of other members.

This is very different from health insurance corporations, which are usually for-profit companies.

Primary Care + HealthShare FAQ IconQ: How is a pre-membership medical condition defined?

A: A pre-membership medical condition is anything where an individual member has experienced one or more of the following: been examined, been diagnosed, taken medication, had symptoms, a personal history of or known increased risk of conditions that may arise or worsen in pregnancy, or received medical treatment, within the 24 months prior to their membership start date. Sharing requests related to pre-membership medical conditions are only eligible for sharing if the condition was regarded as cured or did not require treatment or present symptoms for 24 months prior to the membership start date. 

Note: High blood pressure, high cholesterol, and diabetes (types 1 and 2) are not considered pre-membership medical conditions, provided you haven’t been hospitalized for the condition in the past 12 months, and you are controlling it through diet and medication.

Primary Care + HealthShare FAQ IconQ: Does the membership share costs for prescription drugs?

A: No. There’s no built-in prescription drug insurance component in this membership. However, all members have access to the Zion RxShare program, a prescription discount program.

All HSA for America clients have access to the HSA Advantage Rx discount card program. This can save up to 80% on hundreds of commonly-prescribed generic drugs.

Many clients also use GoodRx, or the Mark Cuban Cost Plus Drug Company.

Primary Care + HealthShare FAQ IconQ: Is Direct Primary Care the same thing as “concierge medicine?”

A: No. Though the two approaches are very similar, they have an important difference: Concierge medicine practitioners usually take insurance; direct primary care providers do not.

Primary Care + HealthShare FAQ IconQ: Can I combine this strategy with an HSA?

A:If you are currently subscribed to a direct primary care membership, you cannot make new HSA contributions. Nor can your employer make them on your behalf. 

However, if you already have money in your HSA, you still qualify for tax-free and penalty-free withdrawals to pay qualified medical and dental expenses.

Primary Care + HealthShare FAQ IconQ: How does health sharing handle pre-membership medical conditions? 

A: Health sharing and health insurance are not the same thing, and they intentionally use different language. A health sharing community doesn’t pay benefits the way an insurer does. Instead, members share one another’s eligible medical expenses.

When it comes to pre-membership medical conditions, health sharing programs typically don’t share those expenses right away the way traditional health insurance does. Instead, they apply a waiting period before expenses related to pre-membership medical conditions become eligible for sharing. The length of that waiting period varies by organization.

With Zion HealthShare, pre-membership medical conditions follow a phase-in period based on the membership start date:

  • Year One: $0 (waiting period)
  • Year Two: up to $25,000 maximum per sharing request
  • Year Three: up to $50,000 maximum per sharing request
  • Year Four: up to $125,000 maximum per sharing request

After four years of membership, expenses related to pre-membership medical conditions are eligible for sharing up to a maximum of $125,000 in a 12-month period. The maximum resets each membership year.
