Meet Dr. Amy Glaser-Carpenter

The daughter of a family medicine physician herself, Dr. Amy Glaser-Carpenter attended Medical School at Midwestern University in Chicago, followed by a residency at Community Health Network in Indianapolis, specializing in family medicine.

She is passionate about serving marginalized communities. She practiced medicine in underserved areas of Chicago as a National Health Service Corps scholarship recipient before relocating to Colorado to serve as medical director for Conejos County Hospital.

As the owner and medical director of Carpenter Family Care, Dr. Glaser-Carpenter continues to expand access to quality care by encouraging prospective members to reach out to her if they cannot afford membership fees to her practice.

She believes in a welcoming and inclusive approach to healthcare, and welcomes patients from all walks of life.

Her commitment to giving back is a hallmark of her practice, reflecting her deep dedication to health equity and access for all.

What is Direct Primary Care?

Direct Primary Care (DPC) is a patient-first healthcare model based on a subscription-like approach.

DPC practices don’t take insurance. Instead, patients pay a monthly or yearly fee directly to their primary care provider.

In return, patients get as many appointments with their doctor as they need to deal with their medical conditions, with no additional copays, no coinsurance, and no deductibles.

Because DPC physicians don’t need to worry about insurance billing and documentation, they can run their practices with much lower overhead than conventional practice.

Membership Fees

  • Adults (26-40 years): $65/month
  • Senior Adults (41 years and above): $85/month
  • Children (0-18 years) with an Adult Membership: $25/month
  • Young Adults (19-25 years) with an Adult Membership: $35/month
  • Individuals (0-25 years) without an Adult Membership: $45/month
  • Adults (26-40 years): $65/month
  • Senior Adults (41 years and above): $85/month

A one-time registration fee of $50 applies to all new members. Membership fees are billed monthly, with the flexibility to cancel anytime.

A reinstatement fee of $200 is required for those wishing to rejoin after cancellation.

Meet Dr. Hoke

Dr. Jason Hoke attended Northeastern Ohio Medical University, graduating Summa Cum Laude in 1995.

He completed residency in Family Medicine at Miami Valley Hospital in Dayton Ohio in 2001.

Upon graduating, Dr. Hoke Hoke co-founded Indian Creek Family Health Center in Oxford  in 2001 with Drs. Nathan Morris and Michael Fain. Over the next ten years, the practice expanded to eleven providers and four offices.

By 2014, Indian Creek Health Center became part of Kettering Health Network.

Driven by a desire to prioritize patient care, Dr. Hokethen founded Hoke Direct Primary Care, focusing on accessible, comprehensive healthcare using the DPC business model.

Dr. Hoke is the recipient of many accolades including, Family Practice Chief Resident, Miami Valley Hospital (2000-2001), Oxford Citizen of the Year (2007), and Oxford Business of the Year, Indian Creek Family Health Center (2007).

Dr. Hoke has been married to Jennifer for over 20 years and has six children. They homeschool their children, participate in church, enjoy outdoor activities, cooking, movies, and cheering for the Bengals.

    • For families with an adult member enrolled, the membership fee for the first two children is $36/month each. Every additional child is added at no extra cost, capping the total monthly expense for all children’s memberships at $72/month, regardless of the number of children.
  • Foster Children:
    • Foster children qualify for the same pricing as biological or legally adopted children under the member’s care, emphasizing Hoke Direct Primary Care’s support for foster care families by making healthcare accessible to all children in the household.

An initial Membership Enrollment Fee equal to the monthly membership fee is charged during the first appointment, along with the first Monthly Membership Fee.

Subsequent monthly fees commence two months after your initial visit. Memberships are flexible, with no binding contracts. You can cancel at any time.
