With DPC, you’ll pay a flat monthly or annual membership fee directly to your chosen primary care provider, granting you access to a wide range of primary care services. Typically, DPC patients get an unlimited number of appointments as necessary to take care of their medical concerns, all for one flat, affordable monthly subscription. There are no co-pays, deductibles, coinsurance, or any other chargers to see your doctor in a DPC clinic.

Derek Farley, D.O.
Dr. Derek A. Farley is a board-certified family physician with over 20 years of experience in emergency and family medicine.
Dr. Farley chose to open a DPC practice of his own because he believes it offers superior access for patients through various communication channels, fosters stronger doctor-patient relationships, and delivers high-quality care at an affordable cost compared to traditional insurance-based models.
Background
After graduating from Ingleside High School in Ingleside, Texas, Dr. Farley attended Texas Lutheran University, obtaining a Bachelor of Science in Biology with a Pre-Medicine Emphasis.
He then pursued his medical degree at the University of North Texas Health Science Center at Fort Worth, graduating in 1997. Dr. Farley completed his family practice internship and residency at Corpus Christi Medical Center-Bay Area.
He then served as the Medical Director of Plano Minor Emergency Center from 2001 to 2003, where he met his wife, Robin Dees Farley, PA-C, MPAS.
From 2004 to 2009, they owned and operated Farley Family Medicine in Little Elm, Texas, before Dr. Farley pursued emergency medicine.
Robin Dees Farley, PA-C, MPAS
Robin Dees Farley is a board-certified Physician Assistant with a Master’s in Physician Assistant Studies.
She holds certifications as a Certified Personal Trainer from the Cooper Institute in Dallas, Texas, and has completed advanced training in nutrition sciences.
As the lead clinician for the Body Sciences portion of the practice, Robin oversees the separate service line of Precision Aesthetics, which provides services such as laser aesthetic treatments and pellet testosterone implants.

NP Care Clinic is a hybrid direct primary care (DPC) model.

- For patients who prefer not to enroll in the membership program, the clinic offers care at slightly higher rates.

Dr. Lezli Braswell, M.D.
Dr. Lezli Braswell, a board-certified family physician with a diverse background in traditional primary care, urgent care, corporate medicine, military primary care, and specialized care for patients with developmental disabilities.
Dr. Braswell earned her Bachelor of Arts in Biology from Texas A&M University and her M.D. from the University of Texas Health Science Center in San Antonio.
She completed her Family Medicine Residency at CHRISTUS Santa Rosa in San Antonio.
Dr. Braswell understands the importance of addressing biological, psychological, social, and spiritual aspects of health.

The Silver Plan is designed for patients who rarely need to consult their primary care physician (fewer than three times per year).
If you think you’ll require more frequent visits, the Gold Plan is likely the most cost-effective option.
Note: certain medical conditions, such as diabetes, uncontrolled hypertension, and a body mass index (BMI) greater than 35, may not be suitable for the Silver Plan due to the higher level of care required.
