Empire BlueCross BlueShield Health Insurance Plan Options

Empire BCBS of New York offers a variety of health insurance plan options for individuals and families both on and off the exchange. For Empire health plans sold on the exchange, these are categorized into metal-tiers. Bronze plans have the lowest premiums while Platinum plans offer the highest premium levels and coverage benefits.

For plans sold off the exchange, they are categorized into:

 

      • CoreCore plans have low monthly premiums but have higher out-of-pocket costs when you get medical care. These plans also have higher deductibles, coinsurance and copays than the other plan types offered by Empire.

      • EssentialUnlike Core plans, you pay less when you get medical care with Empire Essential plan, but still have low monthly premiums.

      • PreferredPreferred plans have higher premiums compared to the Core and Essential plans but offer more health benefits and you pay less when you get health care.

      • PremierPremier plans provide the highest level of health care benefits and have the lowest out-of-pocket costs when you receive covered health care services. Premier plans also have the highest monthly premiums compared to the three plans.
